Denver-Aurora-Boulder, ColoradoAdams, Arapahoe, Boulder, Broomfield, Clear Creek, Denver, Douglas, Elbert, Gilpin, Jefferson, and Park Counties.
Kansas City, Kansas and Missouriprimarily in Franklin, Johnson, Leavenworth, Linn, Miami, and Wyandotte Counties in Kansas; Andrew, Bates, Buchanan, Caldwell, Cass, Clay, Clinton, Jackson, Johnson, Lafayette, Platte, and Ray Counties in Missouri; with a few physicians outside of these areas included.
Memphis, TennesseeFayette, Shelby, and Tipton Counties in Tennessee; DeSoto, Marshall, Tate, and Tunica Counties in Mississippi; and Crittenden County in Arkansas.
New York County (Manhattan).
