Ratings of prices is problematic: Is an Inferior price too expensive or too cheap??? Low/Acceptable/High would be less ambiguous.
Additionally, the "Service Types" in the questionaire do not correspond to the categories offered in the on-line questionaire; discomforting at best as it was an Easter egg hunt. No correspondinf Code Noumbers were provided to assist (Example: Pest controller vs Exterminator (far more categories on-line so ambiguous.)
Just my thoughts as I tried to respond.
